We offer the following IFP health plans:

CommunityCare plans are only available in

These platinum-level health insurance plans

are supported by our tailored network of

doctors and hospitals. They feature $10 or $15

copayments for doctor office visits. There is a 10

or 20 percent coinsurance on most in-network

benefits. A $100, $300 or $500 calendar year

deductible applies on in-network benefits.

CommunityCare POS Gold plans 2015

These gold-level POS health insurance

plans are supported by our tailored provider

network. They feature $10 or $20 copayments

for doctor office visits. There is a 20 percent

coinsurance on most in-network benefits.

A $1,000, $1,500 or $2,000 calendar year

deductible applies on in-network benefits.

CommunityCare POS Silver plans 2015

These silver-level POS health insurance plans

are supported by our tailored provider network.

They generally offer lower premiums and

feature $30 or $35 copayments for doctor

office visits. There is a 20 percent coinsurance

on most in-network benefits. A $3,000 or

$4,000 calendar year deductible applies on

in-network benefits.

CommunityCare POS plans 2015

• Health Net Oregon Standard Gold

(CommunityCare POS) $20/$1,300/$6,350

• Health Net Oregon Standard Silver

(CommunityCare POS) $35/$2,500/$6,350

• Health Net Oregon Standard Bronze

(CommunityCare POS) $60/$5,000/$6,350

Standard CommunityCare gold-, silver- and

bronze-level health insurance plans generally

offer lower premiums. These plans feature

$20, $35 or $60 copayments for doctor office

visits (after the calendar year deductible is

met on the bronze plan). There is a 10, 30, or

50 percent coinsurance on most in-network

benefits. Calendar year deductibles apply.

PPO Advantage plan 2015

• PPO Advantage Gold $25/$500/$4,000

This gold-level PPO health insurance plan

features a $25 copayment for doctor office

visits. There is a 30 percent coinsurance on

most in-network benefits. A $500 calendar year

deductible applies. This PPO Advantage plan

is available with and without pediatric dental

coverage. This does not include Standard plans.

Health Net Oregon PPO Standard plans 2015

• Health Net Oregon Standard Silver Plan

(PPO) $35/$2,500/$6,350

• Health Net Oregon Standard Bronze Plan

(PPO) $60/$5,000/$6,350

These silver- and bronze-level PPO health

insurance plans generally offer lower premiums

and feature a $35 or $60 copayment for doctor

office visits (after the calendar year deductible

is met on the bronze plan.) There is a 30–50

percent coinsurance on most in-network

benefits. A $2,500 or $5,000 combined in- and

out-of-network calendar year deductible applies.
